NiaNexa's foundation is built on specialized knowledge gained from years of work supporting Type 1 Diabetes (T1D) patients in Kajiado County. We are now strategically expanding this patient-centric approach to address the primary components of the NCD Syndemic: Type 2 Diabetes (T2D) and Hypertension (HTN).
The Integrated Health pillar is the clinical anchor of our model. It ensures that health management is not a luxury, but an accessible, consistent reality that enables our beneficiaries to fully participate in our Livelihood and Mental Wellness programs.

We focus on removing the three biggest barriers to effective NCD management in vulnerable communities: access, knowledge, and adherence.
Health Literacy and Self-Management Training
Empowerment begins with knowledge. We shift the responsibility of daily management to the patient by providing comprehensive, culturally-relevant training.
Dietary Guidance: Detailed workshops on affordable, local nutrition optimized for T2D and HTN management, working hand-in-hand with the Seedling Drive to ensure food grown is health-appropriate.
Clinical Adherence Training: Training beneficiaries on the importance of consistent medication schedules, proper use of monitoring equipment, and recognizing symptoms that require urgent care.
Preventative Health: Focusing on smoking cessation, safe alcohol use, and basic hygiene practices to reduce common comorbidities and infection risk.
Targeted NCD Management and Clinical Access
We ensure that high-quality diagnosis and treatment are available where they are needed most.
Community Screening: Running regular, accessible health camps for early screening of T2D and HTN, identifying new cases and bringing them into our integrated care system.
Essential Medication Supply Chain: Securing reliable partnerships to source and distribute affordable or subsidized essential NCD medications (insulin, oral hypoglycemics, anti-hypertensives).
Referral Pathways: Establishing clear, subsidized referral pathways to specialists (e.g., ophthalmologists, cardiologists) for managing NCD complications that exceed primary care capacity.
